Valid every 6 months with routine care. Patients must complete two cleanings per year, be in good oral health, and be clinically eligible for whitening as determined by a licensed Ideal Dental provider following a required exam. 21 years or older. Cannot be combined with other whitening promotions or discounts. Replacement gel outside of the routine schedule may incur a fee. Office reserves the right to modify or discontinue the program at any time.
